    To find a PHP (HMO SNP) network provider, facility (like a hospital or urgent care center), or a pharmacy, you can: — OR — If you want a copy of any of the above directories, please ask Member Services to send you a copy.
    To report any errors in the PHP provider or pharmacy, please send an email to florida.directory.errors@positivehealthcare.org, or send a message using our web form. You may also report directory errors by calling Member Services at (888) 456-4715 (TTY 711). Agents are available 8:00 am to 8:00 pm, seven days a week.
    PHP is an HMO plan with a Medicare contract.  Enrollment in PHP depends on contract renewal.
