California PHP

Thank you for considering PHP (HMO SNP). PHP is a Medicare Advantage “Special Needs Plan (SNP)” that includes prescription drug coverage. The plan is a SNP because it is designed around the health care needs of people living with HIV. This is an important consideration when choosing a health plan.

To be eligible to enroll in the plan, you must be a Medicare beneficiary enrolled in Medicare Part B, have an HIV/AIDS diagnosis and live in Los Angeles County.

PHP’s goal is to keep its members healthy through routine primary care doctor and specialist visits and tools to help members follow their doctor’s care plans. Each member is assigned his or her own registered nurse care manager who acts as a personal health coach and helps coordinate care. The plan makes sure every member can reach a nurse 24 hours a day, seven days a week.

PHP offers other features and additional benefits at no cost and a prescription drug program that are geared to people living with HIV.

Every year, Medicare evaluates plans based on a 5-star rating system. PHP is rated 3.5 stars for 2021.

PHP is an HMO plan with a Medicare contract. Enrollment in PHP depends on contract renewal.
