California PHP

Although Medicare is a Federal program, PHP (HMO SNP) is available only to individuals who live in our plan service area. To remain a member of our plan, you must keep living in this service area, which is Los Angeles County.

We also offer PHP in Duval, Broward and Miami-Dade Counties, Florida and Fulton County, Georgia. However, there may be differences between the plans we offer in each state. If you move out of Los Angeles County to Duval, Broward or Miami-Dade County, Florida or Fulton County, Georgia, and you wish to remain enrolled in PHP, you must call Member Services so we can arrange for you to transfer your enrollment to the Florida PHP plan.

If you move to a county or state outside of our service area, you cannot remain a member of our plan. Please call Member Services so we can assist you with disenrollment from the plan. When you move, you will have a Special Enrollment Period that will allow you to switch to Original Medicare or enroll in a Medicare health or drug plan that is available in your new location.
